This page provides download access for our Confidential Estate Planning Information Form.  There are two versions of the form, one for couples and another for single persons. For each of these versions, there are three variations, one for printing with Adobe Reader, a second for computer processing with Microsoft WORD and a third for computer processing with WordPerfect.

Adobe Reader:  This version is for those wishing to print the form and complete it with pen/pencil.  It requires Adobe software to use.  If you do not already have Adobe Reader, you may download a free copy at www.adobe.com .  Once you have displayed the form on your screen, you may save the form to your disk and then print it for completion - or may print the form directly from your browser.

Microsoft WORD:  This version is compatible with WORD Versions 6 and higher.  The form may be saved to your hard disk and filled in using your version of MS WORD.

WordPerfect:  This version is compatible with WordPerfect Versions 8 and higher.  The form may be saved to your hard disk and filled in using your version of WordPerfect.

To use the WORD or WordPerfect forms, click on the appropriate link below.  When given the opportunity, OPEN the form, save it to a directory of your choice and fill in the requested information with your word processor.  When the form is completed, you may print and mail a paper copy or send the file to us as an attachment to an e-mail message.
